Behavior Reflection Sheet
I use this behavior reflection sheet when students are sent to the guidance department after bad behavior. I have found it extremely useful as it is reflective in nature rather than punitive. It also includes a section where students are asked to reflect on their positive traits that can help them to make better decisions in the future.

Behavior Tracker - Weekly/Daily CUSTOMIZABLE
Use this weekly/daily behavior tracker to incentive behavior through a point system. Use it as a general behavior tracker, or to set goals with your students. Set the students three main goals, track how they do with each subject, and review the day.
