Retake Form- Test/Quiz/Assignment
Use this retake form to help students prepare for a test, quiz, or assignment retake.
I like to conference with students and talk through the reflection questions together. Students write down what was confusing or what they didn't understand, what they did (or didn't do) to prepare, what they are going to do to be ready for the retake, and the date they will be ready. Then I staple the retake sheet to their test, quiz, or assignment and send it home for parents to review and sign.
