This is a generic permission slip template for an after school book club that I made. This template works for most book clubs and after school clubs with a little bit of editing. It includes:

- Emergency Contact Information

Bookmark Book Report
The Bookmark book report is a tool to enhance reading comprehension for fictional books.
Key areas of emphasis are deeper understanding and articulation of the main idea / conclusion, vocabulary, and contextual information like character traits, events and settings.
You can print two bookmarks on one page and cut it along the border, fold and glue the two sides for a summary that can be used as a bookmark while reading the book and also for jotting notes on the book mark.
