"Behavior" contract daily feedback form with rubric outlining the different levels of attainment. Designed to track explicit and measurable behaviors that parents and educators want to "encourage" as opposed to subjective or negative behaviors to avoid. Rather than "doesn't distract during class", "uses class time wisely" with a level score for "excellent", "fair" and "needs improvement".
