FREE P.B.I.S. Think Sheet
The school where I teach is transitioning into using P.B.I.S..
I created this Think Sheet for our staff to use. We make copies of the Think Sheet to keep on hand.
We copied pages 2/3 in a front/back method and laminated them. We only needed one per classroom. Get to Know You
Are you a primary teacher looking for a way to get to know your new students? This is a fun "Get to Know You" form. It can be placed in the front of a student binder. It is also a great paper to leave in your sub folder so that the sub can have some fun background information on each students for easy bonding. I also encourage teachers to bind these together and give a mini book to the special area (Art, Music, PE, technology) teachers as an introduction to your class.

G-PAR Behavior Plan Form
The G-PAR Form was developed in response to my students needing a way to set personal goals for their academics or behavior. G-PAR stands for Goal, Plan, Action, and Result and is a component of William Glasser's Choice Theory. This form also includes a first step of "reflect" which allows students to first think about what behavior they want to change before creating a goal. It provides teachers with an option to set unofficial behavior plans for student self-improvement.
