This information card is a parent survey for the first day of school. It is a quick card that I would print out on card stock to supplement parent information cards that were kept in the office since they were often hard to access. Information includes contact numbers, siblings, student medial needs and academic needs. For Spanish speaking parents only, it can be completed quickly on the first day of school when students are dropped off. Make sure to have plenty of copies ready and pens/pencils
