PRINTABLE Speech Therapy log for data collection and progress monitoring

Looking for a fast, simple way to do your therapy logs? I use this quick and simple template to track attendance and progress towards speech goals.

I print mine double sided to save paper and you can print off page 2 for pages 3 and onward. Once the annual review comes up I print page 1 again so that I can refer to the new updated goals.

Print pages 1-2, and simply add copies of page 2 as needed until goals are updated at annual review.

Edited on 2/7/2019: A sample screenshot can be viewed on page 3, added to demonstrate how I use this product for my students.

This file is in Word format, so page 3 can be removed and column options can be formatted to suit your therapy setting/caseload (for example, if you only work with very young students, you may prefer to remove "worksheet" under "activity" column).

Bonus use: the top section of page 1 can be copied and used as a classroom teacher 'IEP cheat sheet' if desired!
