Hygiene Self Assessment

This self assessment provides students an opportunity to explore their personal hygiene beliefs and behaviors. The assessment also allows the teacher to gain an understanding of the student's beliefs and behaviors. The information gained from this assessment allows teachers to plan individualized instruction for a hygiene unit. Students examine how often they perform hygiene tasks including brushing teeth, washing hands, showering, wearing clean clothes, etc. The assessment can also be taken by teachers and parents in and used in transition planning.
