This is a handout used for reading log journals. Students staple it in their journals, and use the response questions to respond to their reading. The requirements that are listed are that students must read a total of 150 minutes per week, and students must respond to a prompt for every 30 minutes of reading. Also included is a grading rubric sheet that has 4 rubrics per page. When I grade students' journals, I staple the rubric right in their notebook.
