Vaxx Trak - Immunization Tracker for USA Childcare centers - CDC Edition

Simply put, this is a carefully formatted Excel document that provides automatic tracking for state required, early childhood immunizations.  Vaxx Track is an Early Childhood Immunization Tracker providing automatic updates for required immunizations after an initial entry of a students’ information and current immunizations. The spreadsheet will complete the daily, weekly, monthly and yearly monitoring of the specific state required immunizations.   

At one month prior to an immunization becoming due, the cell will turn yellow.  This is a visual cue that could be used by the administration to send out a reminder for upcoming immunizations. 

After the required date for the immunization has passed, the cell will turn red and display "LATE" in the cell.  Upon receipt of the updated immunization records, simply enter the dates in the "LATE" cell and sequential countdowns will automatically resume. 

There are considerations made for affidavits, with the date cell turning red after the two year time-frame has expired.  Finally, there is a column at the far right of the form for tracking notes and comments (i.e. date notes were sent, parent contact dates, etc.). 

   This spreadsheet was a collaborative effort with KELEE Consulting, LLC, specifically created for USA Childcare centers that are not big enough to have a childcare management system and are managing the immunization information themselves. This spreadsheet provides a way to simplify this tedious, time consuming, state required and never-ending task.
