Unit Data Form: Monitor Progress & Plan Intervention and Differentiation

This Excel worksheet automatically color codes scores as you enter them to show baseline and target benchmark levels. This is a great tool to monitor progress within a unit or concept and to easily track students' needs for intervention and differentiation. Since this form color codes score ranges automatically you will always have necessary data for student growth without taking any more steps than entering your grades!

Editable data fields include Pre-assessment scores (color coded to show baseline benchmark levels) and Post-assessment / Formative assessment scores (color coded to show target benchmark levels). Formative assessment scores use lighter coloring to differentiate from the Pre-assessment and Post-assessment data. All of these fields and color coding can be edited as detailed below.

Easy to follow directions (with pictures of the steps) are included for how to change score ranges and colors to make this form entirely customizable to fit your needs. Directions are also included for how to easily make pull down menus and how to set them to color code automatically too. With all the push to show and use data these directions will make it a lot easier!!
