Transition planning for IEPs is a process to help students with an Individualized Education Program (IEP) decide what they want to do after high school. This is a required part of a student's IEP by the time they turn 16 and most districts require you to begin working on it by age 14.

To develop the transition plan, the IEP team works with a student to identify strengths and interests, as well as plan for supports for areas of weakness.

Use the student questionnaire that works best for your student. This can be an individual activity that the student completes on their own, or they may have accommodations to complete the surveys including reading the questions, clarifying the content or writing the responses.

Send the parent survey home and request it back before the IEP.

Use the student invitation to give to the student to ensure and document that they know about the upcoming meeting.
