Student Successful Re-Entry Plan Template

How do you handle a student re-entering a setting after an incident? It can be tough to teach new strategies or create a plan for students who have struggled to demonstrate expected behaviors. This template is a plan for students who need scaffolding before re-entering a setting that they are not yet prepared for. The plan has an outline for staff members, data collection, goal monitoring and goal setting. This is not exclusive to behavior or special education students it is a document that can be used as an intervention to address any need for students from aggressive behavior to functional skill building.
