Student Portfolio - Special Education

These are the portfolio's my work is putting together for next year. I am leaving it in a powerpoint so you can change anything to meet your students needs. We add a 3 hole plastic business card sheet (.39 each) inside each one. The students love collecting business cards from their doctors, dentist, eye Dr, regional worker and any other resources they may need. We also have added 3 plastic picture holding sheets. The thought is that the students will be less likely to toss the portfolio after graduation if there are pictures of them in it. I added papers that state this inside the document. We are an adult transition program for students up to the age of 22 so some material may have to be adjusted for high school students. This portfolio will stay in the classroom, brought to IEP's to show sample work in the back, and passed to the next years teacher. When the students graduate the portfolio will go with them to take to their next adult placement. Please look over the preview carefully before buying. *update* I added "Personal Assessment" forms that can be used with older students instead of the Parent/Guardian Assessments if you would rather the students fill them in.
