Student Organization/Behavior Checklist

This product is meant to be used to help students with ADHD (Attention Deficit Hyperactivity Disorder), ADD (Attention Deficit Disorder), and Specific Learning Disabilities on improving their Organization Skills / Behavioral Management which is a common struggle for students with ADHD, ADD, and Learning Disabilities. This product requires the student to evaluate themselves on a Yes/Needs Improvement/No scale on common classroom procedure of a neat backpack, completed homework, attitude, organization, handwriting, etc. This helps the student improve on a number of skills such as self-evaluation, self-advocacy, organizational skills, time management and many more. There is also a spot for each teacher to leave additional comments on students' overall behavior in the classroom that day for each period and a place for both their official teacher and parent to sign indicating they have reviewed the document.

Also included is a checklist for students to use in the morning before school and the afternoon when they get home from school this is to help them set a routine of making sure they have all their items for the start of the school day and making sure they come back with them. I suggest laminating this document and giving the student a dry erase marker so they can use the same sheet every day.
