I have developed a basic Speech/ Language Assessment report template that I use for all my kiddos. This template makes me feel secure walking into any IEP meeting, knowing that I accounted for every possible resource, and obtained and analyzed all information appropriate to making a clinical and legal decision.
Things to consider putting in your reports: previous assessment scores, ELL info, history and background information, classroom observation, spontaneous language abilities, test scores, other behavioral factors, and your state license number.
This is a Triennial report, meaning that I ONLY assessed areas that the child qualified for and is currently working on, as opposed to doing the FULL comprehensive evaluation of 2 Language assessments etc.
This is a real report that I wrote. All names and identifying information have been changed to protect the identity of the student.
