This template is for The Social Language Development Test -Elementary: Normative Update (SLDT-E: NU). This test evaluates social language skills. The test is comprised of four subtests, each focusing on different skills. The subtests focus on taking someone else’s perspective, making correct inferences, negotiating conflicts with peers, being flexible in interpreting situations and supporting friends. It is designed to be used with children ages 6- 11 years, 11 months years.
The template can be inserted into the body of your report. The template includes an overview of the test and a format to report on all 4 subtests and the Social Language Index Score. There are two easy to read tables included. The template is 4 pages long including the cover sheet.
** Full Disclosure- This report template was written by me, as an elementary school-based Speech-Language Pathologist. It can be easily adapted to other tests and specialties. I have no affiliation with ProEd. This product does not replace the need to buy the official and updated batteries of assessment and protocols from them. This report template should be used as a guide/ time saver to assist in writing your report. Please ensure that this report is completely modified to adapt to your individual students.
