I created these logs and calendars after I was audited my first year! This is an easy way to keep track of services and to keep you organized! Great for showing your evidence of services! Also great for Medicaid billing!
Each form includes a key to quickly document absences (SA=student absent, SRA=School Related Absence, H=Holiday, ect..), therapy formats (I=individual, G= group, INC= inclusion, ect..), and and progress: (W= working, R=regression, P=progress, M= mastered etc...). They were designed to document fast and efficiently! The calendar follows a similar key! If you don't like my codes, you can now write your own in with the "editable" versions! T
