Description
This set of Early Intervention Articulation data collection sheets for the Parent and Teacher makes taking speech therapy documentation a breeze. My teachers were nervous about tracking data when completing articulation interventions. In this product, you will find forms you will send home with the parents and give to the student's teacher for all phonemes in the initial, medial, and final positions of words.
The phonemes are broken down into the initial, medial, and final positions of words. So, there are 10 words for each sound position. For example, there are 10 words for ch initial, 10 words for ch medial, and 10 words for ch final. These are all on separate pages. See the preview above.
The product has documentation for six weeks of intervention; if the committee decides to have interventions for longer than six weeks, just make multiple copies for the teacher and parent.
The teacher and parent will follow the directions on the form to complete the data table. Use the data tables at your next intervention meeting to decide whether your interventions work!
