Description

Updated: For 2016-2017 School Year! new calendar options added, new fonts and borders!

Want a better way to track data? These 12 sheets are ready to print and aligned for 3 hole punch! Includes everything you need to create a speech portfolio for each student!

CONTENT:
--15 printable sheets for a Speech and Language Therapy Portfolio.

-- Info sheet

--Title Page (page 2): Students write their name, draw a picture and fill out the bottom portion. Year was left blank so you can reuse!

--Inspiration Sheet (page 3): At the beginning of the session pick one of the phrases to say aloud to remind students “You CAN do it!”.

--All About Me (page 4): Students complete this page in the first days of speech, then share as a group.

--My Goals (page 5): With Speech-Language Pathologist’s help, students write summaries of their goals for the year. Review them often and see if students can memorize their goals.

--Sticker Incentive Sheets (pages 6-7): Depending on whether you provide stickers/stamps for hard work, attendance, homework or all of the above, these sheets provide a place to do that.

--Attendance Sheet (page 8-10): Have students circle today’s date every time they attend a session. This will provide proof that your students attended, services are being met and provides a back-up for your billing. Includes a portion for you to sign at the bottom to make it more official. 3 different sheets! One for M-F, 1 full week 2015, 1 full week

--Graphing Progress (page 11): Print multiples. Each student has 1 page per goal, and depending on what goal was targeted they track their own progress using the bar graph. When you run out of room, just print more.

--SLP Notes (page 12): A place to quickly write down any additional notes on the session. For example: activity used that day, progress notes, or behavioral notes. Later these notes can be added to student’s IEP files.

--Speech Session Log(page 13): A place to log lesson plans, therapy materials used that day, goal(s) worked on, individual/group session, reason a student missed the session, and behavioral notes. This my shortened version of a SOAP note!

--My Speech Log (page 14): A place for older students to log what they worked on that day (put the responsibility on them!). Take 1-2 minutes to log what activities they completed and goals targeted.

--Data Collection Sheet (page 15): A place to collect data on the goals you targeted in your sessions. Write the date, the goal number or a short form (e.g. past tense verbs), use your own preferred symbols to track correct/incorrect, prompted etc. Then calculate the % correct and add notes as needed.
