My Speech Language Report Editable Templates Case Studies MEGA PACK is a collection of 12 reports I have written with real cases across a variety of S/L domains. I have worked in Low socioeconomic status/ Title 1 schools with English Language Learners for 9 years. This tool is essential for any new SLP or CF in a public school setting or private practice, any SLP Grad student, or for anyone who wishes they had someone to collaborate with on tough cases.

This pack includes 12 real full evaluations and thorough case study reports that are in Word 2013 and fully editable to be used as a template and for reference for your most complicated cases. Each report is 3-5 pages. The ages/grades included are elementary (Prek-3rd ) but can be used up to 5th grade.

The cases I have included describe the influence of Spanish on English language expression and phonology, how to use BICS/CALP in a report, Portfolio style assessments for ELL kiddos, how to describe African American English dialect and differentiate from AAE vs. a language disorder, verbal and non verbal kiddos with autism, how to assess and report voice and tongue thrusting in a public school setting, how to write a “review of records” without having to do a full evaluation, and how to determine when articulation impairments are affecting expressive language skills.
