3 Google Form Surveys are included to gather data from parents, teachers and the students themselves. This product will provide you with the documentation you need to communicate about a student at the IEP table.

How to use:
This product is a Google Form and will require you to have a free Google account. Once you purchase the product and click "download", you will have your own copy of the form to edit as you see fit.
Attach the form in an email to the parents. To do this, click the SEND button in the top right corner. Make sure the letter (email) icon is selected and type who you are sending it to. In the message part, type the student's name and check the "Include form in email" box.
Then hit "send".
Once the parent has completed the form, you can see their results by going to "Responses" at the top of your form.
