Special Education IEP Transition Assessment Interest Inventory
This is a (7) page PDF file that meets the Transition requirements for IEP Development.
“All About Me” is easy to complete because the questions are designed in a “complete the sentence” format and check boxes.
Below are some examples of a few of the “complete the sentence” questions that are included in the assessment:
“When I have free time, I like to….”
“If I could choose between playing a video game, watching my favorite TV show or using a computer, I would choose…….because…..”
“My family and friends would describe me as …”
“I think I am best at …..because….”
“I learn best when….”
“When I finish school, the job I hope to find is ….”
There are (5) sections that are “check all the boxes that apply to you”
The check box sections and some of the choices include:
“I think that I am”
A few of the choices are: responsible, artistic, friendly, shy, etc.
“I prefer”
A few of the choices are: to work outside, to build things, etc.
“I usually”
A few of the choices are: finish a task I start, frustrate easily, etc.
“I am a good at”
A few of the choice are: organizing things, using my hands, yard work, etc.
“I think I struggle with”
A few of the choices are: pay attention, following directions, math, using time wisely, etc.
Although this is considered an informal transition assessment, I use it for all of my students because it provides great information for developing IEP profile pages and behavioral goals.
