This video contains a tutorial on how to use a pre-made, Microsoft Word evaluation report template to help expedite your report writing for initial evaluations and reevaluations. The Microsoft Word template consists of eleven sections with pre-made report templates including: Reason for Referral for both initial evaluations and reevaluations, Background History, General Health, Cognition, Academic Achievement, Phonological Processing, Fine Motor, Social/Emotional Status, Executive Functioning, Adaptive Behavior, and Classroom Observation.

Pre-made assessments templates provided include: KABC II, WISC V, CTONI-2, KTEA-3, WIAT 4, CTOPP 2, FAR, FAW, KeyMath 3, GORT 5, TORC 4, TOWRE-2, WIST, and ADOS-2 for Modules 1, 2, an 3.

Pre-made rating scale templates provided include: BASC 3, DP-4, SAED 3, ASRS, RCMAS-2, Conners 4, BRIEF 2, ABAS-3, and Vineland 3.

The Microsoft Word template also includes auto-populated fields for the student's name, pronouns, grade, school, school district, primary language, parent's name, general education teacher's name, and special education teacher's name, which autofill throughout the evaluation report. This will save you time on report typing and having to proofread reports for errors.
