Simple Yes/No Data sheet

This data sheet is a simple sheet that can be used to take data on a variety of targets on one sheet. You can write the student's name on the top of the sheet, list the overall goal at the top and list the targets vertically. The date is placed at the top of the column where each column represents the day. I use this data sheet in inclusion classes where I can take data on observable objectives that are easy to check off throughout the day (ex. raises hand, attends to speaker, follows multi-step directions, etc.). At the end of the week or after a few days, I can look at this sheet and see which areas the student is successful and what we need to continue to work on.
