Description
This product is great for providing feedback to graduate students/interns. There are two forms included in this download - one for evaluation sessions and one for therapy sessions. Feedback forms include a simple rating scale in addition to sections for notes and comments.
It can be used by both the graduate student and supervisor to engage in self-reflection and feedback discussions.
