Doing a staff Secret Santa exchange this year? You need this questionnaire! Thorough, but well-designed and cleanly laid out, this questionnaire will give you information on your giftee's favorite foods and interests, plus tons of other information that will help you choose the perfect gift(s).

This form includes:

• My Favorites: Favorite color, favorite snacks, favorite movies, favorite sports teams, favorite stores...it's all here.
• Holidays: Two quick questions: what holiday(s) your giftee celebrates, plus their favorite holiday tradition or memory.
• Memories: Two quick questions about your giftee's childhood that may give you some more insight into their personality and preferences.
• My Wish List: A place for your giftee to list items they may want for their classroom or for themselves.
• I don't like/I don't need: A place for your giftee to steer you away from gifts they won't like.
• Preferences: A list of common gifts for your giftee to indicate that they would like, or to politely decline.
• Social Media: An optional place for giftees to list social media profiles where their Secret Santa could find more information (Facebook, Twitter, Instagram, Pinterest, other.)
