Description
This is an editable word doc student needs assessment for the beginning of the school year. I have used this for several years in google forms to get to know my students, screen for groups, plan out the school year, and assess school culture.
Upon request I will share a duplicate of the survey ALREADY ON google forms, your email will need to be provided in comments. [must be a gmail to view your survey results]
