This is an easy-to-use Excel workbook consisting of 4 spreadsheets. It allows the SLP to record information about one individual student and their therapy sessions and then calculates and displays important information, including the following:
* Progress toward therapy goals
* # Student absences
* # SLP absences
* # Service minutes received per month
* # Times each goal has been targeted
* Student's current age (in years, months, and days)
* IEP and Eval due dates
HOW TO USE
*Student info and therapy data are entered in the Therapy Log sheet. Data is automatically calculated and displayed in the remaining spreadsheets (Dashboard, Goal Progress Chart, and Therapy Data Tables), where it can be viewed and printed.
*Tips on data entry and formatting are given throughout the spreadsheets and are indicated by red in a cell's upper right corner. Click on or hover over these cells to see the tips (these will not show up on print-outs).
*Click on the tabs on the bar at the bottom of the page to navigate from one worksheet to another.
THERAPY LOG
The Therapy Log is the only sheet that requires data entry. Fill out student data and goals, using the format mm/dd/yyyy for dates.
DASHBOARD
No data entry is required. This sheet gives quick reference to important stats, such as the student's current age, upcoming IEP and Eval due dates, and monthly goal progress.
GOAL PROGRESS CHART
No data entry required. The Goal Progress Chart automatically updates to show the student's progress on each goal for every month of the school year. You can choose which goal/s to display by selecting them on the right side of the chart.
THERAPY DATA TABLES
No data entry is required. The Therapy Data Tables sheet is for viewing and printing. The numbers are automatically calculated based on your input in the Therapy Log.
This sheet shows in table form the following: Goal accuracy by month, # of service minutes per month and whether the student's monthly minimum service time has been met, # student absences by month, and # SLP absences.
FORMULAS
This sheet contains the formulas necessary to make the spreadsheets function. Altering this sheet in any way may prevent the spreadsheets from working.
