Reading Interest Survey and Book Review

One of my opening week activities is to have my kids fill out this Reading Interest Inventory (usually while kids are taking turns doing their first book shopping) to learn a bit about their tastes and habits.

Every time one of my students completes a chapter book, I ask them to fill out one of these simple, but engaging, book reviews. It helps assess a few areas quickly.

Do they know the genre? Can they visualize a scene from the book? Are they able to give a brief summary?
