What educators are saying
I needed a tool to keep open communication between my son's middle school teachers and me. It needed to be easy enough that it wouldn't be a burden on the teacher, but would give me enough information to keep things on track. This resource works well!
This was a great starting place for making a behavior plan for a student - I'm appreciative of the ability to edit to meet the needs of my student.

- State the behaviors in a way that tells your student what you want them to do. For example, use appropriate language, raise their hand to speak, etc.
- Explain to your student how the log works and explain the specific behavior expectations that you are targeting. Explain that you will be sending this home for a parent/guardian signature each day and that they are expected to return it to you daily.
- Communicate with parent/guardian about the log so that they know to sign and return the log.
- Communicate with any other team members who will be documenting behaviors on the log, such as other teachers.
- Keep enough copies of the charts on hand in an accessible area.
- It can help to keep the logs in a folder.
- Consider stapling the logs together at the end of each week and keeping them in your student’s file.
- The charts can come in handy at Parent Teacher Conferences, IEP meetings, etc.
