Description

Save yourself tons of time with these fillable forms! First is a Present Levels Input Form. This is a request form to give to the gen-ed teacher prior to the IEP meeting. The second form is an IEP Snapshot that you give to the gen-ed teacher after staffing. This gives them an overview of the student's services, goals, schedule, and accommodations. It's a helpful reference that they can view at a glance without going through the lengthy IEP paperwork.

Both forms are provided in a fillable PDF that you can type the information into and a print-and-write PDF if you rather handwrite the information. The fillable input form can be emailed to the gen-ed teacher, filled out online, and then emailed back to you.

The input form is also provided in an editable Powerpoint in case you need to customize it.
