Data Collection Sheet

Simple basic assessment sheet (one page front to back!) made to assess students once per quarter. Create your own color code at the top using four different colored highlighters and track student progress throughout the year!

Assessment includes:

• Uppercase letter identification
• Lowercase letter identification
• Letter sound identification
• Rote counting
• One-to-one correspondence
• Subitizing (identifying quantities 1-5 without counting)
• Numeral identification 0-20
• Basic shape identification
• Basic color identification

Excel Spreadsheet

1. Enter the names of your students (up to 16 for both AM and PM classes).
2. Enter the data you've collected on the data collection sheet.
3. The data will be automatically color coded to show growth in the following order: red, orange, yellow, green.
4. A graph will automatically generate so you can visually see how many students are in each color group!
5. Click the tab at the bottom of the spreadsheet to view the graph once you've entered the data.

The spreadsheet is very helpful for planning small group instruction and can be used to show student progress!
