Portfolio Data Collection

This is a data collection sheet I made for the students in my special education class that take the TCAP portfolio each year. If you are a teacher who completes this assessment with your students each year, you know how difficult it can be to keep data organized, especially for those students who cannot complete worksheets and must rely on observations/data collection.

Even if you don't have students who complete the Portfolio-Alt assessment, this could be use for pre-k/k teachers.

It is generic in some areas in terms of actual goals so that you can customize to each student. There is a specific area for money and shape data collection. It is so simple to use, that my students even take data using this sheet.
