Parent Open House Information Sheet - Middle School/High School

This is the parent survey I give out during our open house/parent night at school. I wanted something that would help me learn about the students in my class and just as importantly, how their parents see them. Asking parents about their students' strengths and weaknesses has been a helpful way to show my parents that I care about their sons/daughters. I also have found it useful to see if the parents' perception of their students' abilities and work ethic are accurate. As a high school teacher, I don't need as much information from the parents as many elementary teachers, so this form is short. I put two on the same page so they don't waste paper.
