I have developed this survey over the past 30 years through my experiences of working in both clinic and school settings, with a verity of clients, and around wide-ranging food issues. Using this tool gives me a much clearer picture of what is or is not happening and helps the family and me to target realistic and measurable goals. A checklist format makes this easy and fast for family members to fill out yet gives the therapist precise information.
