Middle School ELA Parent Survey

This document is sent home with the course outline (syllabus) and is used to find out pertinent information about the students, but from the parents, at the beginning of the year. For example, one of the questions asks if students have internet access at home, which helps the teacher in making accommodations for certain students, but it also gives insight as to which students shouldn’t have any excuses as to why their online homework isn’t done. All the information from this letter can then be put on a class roster as a checklist item for each student and then the teacher can easily see information about each student in one place, rather than having to look it up.
*It is LGBTQ inclusive, as well, so that those students know that they are welcome and respected.
