Description

This EDITABLE math student profile consists of ten statements and four brief questions; its purpose is to determine a student’s strong and weak areas in math. Students are asked to complete a ten statement math survey by simply checking the box that describes their personal views about learning math. The boxes are labeled as: 1) Strongly Disagree, 2) Disagree, 3) Agree, and 4) Strongly Agree. After the survey is completed, students complete four short answer questions.

Since every math class is unique, using this short profile at the beginning of each semester helps an instructor determine a student’s attitude towards math and the perceived capabilities the student has. Sometimes, I give the same profile at the end of the semester to see if the students’ perception of themselves has changed at all. In addition, this profile is valuable as an individual pre/post evaluation to place in the student’s portfolio. Regularly, I ask the students to select one area in which they plan to improve during the semester. Then they write a short paragraph of how they hope to accomplish this goal which I staple to their profile.

Since this document is not saved in a PDF format, you can adapt it to meet your needs. The first three math statements as well as two of the questions are on the preview version.
