Lesson & Classroom Observation Form
Description
Pre-service teachers can use this form when assigned hours for field experiences, observations, student teaching, or internships. Students in high school may also want to use this as a form for job shadowing a teacher. Additionally, mentor teachers can use these forms for their student teachers or novice teachers.
The questions posed on the sheet are as follows:
-What was the objective of the lesson? How do you know?
-How did the activities of the class accomplish and support the objective?
-Give a synopsis of the class period.
-What would you have done differently?
-What did you appreciate and admire about the lesson and instructor?
-Describe the appropriateness of the activities for the developmental age of the students.
-What lesson format was used? (Madeline Hunter, 5E, EDGE Protocol, Charlotte Danielson, Flow Learning, etc.)
