I have included my Ultimate Reader’s Notebook forms with these mini-lessons, which I hope that you find helpful in setting up the notebooks with your students. You will want to pick and choose which pages you want to use. I also included divider pages, which you can insert into page protectors and then use different colored sticky notes for tabs that stick out. This makes it really easy for students to find each section.
The student and teacher reading data sheets are from my Ultimate Teacher Data Notebook and my Ultimate Student Data Notebook. If you are interested in the data sheets for other subjects, you might like to purchase these products at my store.
