Yes! The self-grading Google Forms quizzes include the same questions as the paper-based assessments. This enables you to differentiate how the assessments are presented to students depending on needs!

Is there a version of the diagnostic assessments WITHOUT the grade-level title printed at the top?

Yes! The toolkit now includes a discreet version of the diagnostic assessments without the grade band printed at the top. You can use this version of the assessments to assess students who may be above or below grade-level standards and sensitive about their abilities.
