Description

Save time by making data keeping easier by using these efficient and informative data templates for goal areas in Reading, Writing, Math and Behavior! By keeping this basic data weekly, it is easy to see where students are at on their goals, how your instruction aligns with student's IEPs, which objective they are currently working on, and how to anticipate and prepare for upcoming IEPs.

In this resource you will find:

- Reading, Writing, Math and Behavior data sheet templates that last the entire school year. When filled out, they clearly show tasks that were completed with the student by week, how that task aligns with their IEP goal, and how they are progressing towards their goals. Checklists help with efficiency, while spaces for notes allow for further commentary to help create a fuller picture.

- There are 11 packets of data sheets, about 48 pages in length each. These are individualized by the month the student’s IEP is to be held. Each month is split up into trimesters of 13 weeks (one will have 14 weeks) with clear indicators as to when the student should have met each short-
term objective and when to move on to the next objective (or to their annual goal). Each month has the same Reading-Writing-Math-Behavior templates, (in that order) which allow for consistency and are about 48 pages long, which will last an entire 40 week school year.

- Headers on each page have a place for the goal to be inserted for easy reference.

- Three file options; PDFs (that have color and clipart) or an editable DOC file version. (To protect fellow seller’s copyrights I cannot include the unique fonts and clipart in these DOC versions, both doc.x and doc so one version should be compatible with your version of Word) The content is the same.

I print out a packet for each student and keep the forms in his or her individual work folders. (They have one folder per subject). When they are to move on to the next short-term objective, I swap out the pages, file the data, and write the current progress report. It is made to be clear-cut and easy to use for both my staff and myself. The on-going commentary I write on my student’s data sheets is especially useful to use during conferences with parents. My notes allow me to speak more in-depth about their child’s
progress. The forms also allow for anyone walking in or through my room to see that my instruction 
is always aligned (in some way or form) to my student’s IEP goals and exactly where they are at in meeting their goals.

It is my hope that this resource makes your job a little bit easier, freeing you up to enjoy those sweet kiddos in all their wonderfulness and also create some breathing room to relax when you get home and take care of yourself. Happy data keeping!

*** My students are in 3rd through 6th grade and working on academic skills ranging from a pre-acadmeic level to a third grade level. With one unique exception, I use this "generic" form for all students, regardless of level.

I created these forms using a 40 week school calendar starting at the beginning of September and going through the middle of June, with only two weeks of instruction in December. You can adapt as necessary to fit your school's schedule.
