I have this checklist printed for each student in my conferring binder. I have the first 2 pages (levels Pre-A to V) printed on one page, front and back. I will only print page 3 for students who surpass level V. It's up to you how you choose to organize your data!
In the download, I have also included links to fluency rubrics that you can use with this checklist as well as pictures of my conferring binder.
