This Extra Credit Application can be used to stipulate the conditions, agreements, and requirements for extra credit assignments in your classroom. This form is suitable for any subject area and is beneficial for students and teachers alike.

Use this form to prevent last-minute extra credit assignment requests. This form will clarify to students that they must apply for extra credit and will encourage proactive learning and goal-setting. This form also supports meta-cognitive learning by prompting students to reflect on their reasons for requiring extra credit.
