Description

This informal fluency assessment for stuttering will help you collect data, write the evaluation report, and monitor progress in speech therapy! I use it during initial speech therapy evaluations and annual IEP reviews. Checklists are included for the teacher and the student to determine self-awareness and documentation for educational impact.

What's included?

  • Fluency Informal Assessment - I put dots for each fluent syllable and (-) dashes for each stuttered syllable/moment. Add up your stuttered syllables and divide them by 150 to give you the percentage of disfluency. Compare this to the included key to determine the severity rating. You will check off the type of disfluency and the secondary characteristics noted. Examples are given for each characteristic on the assessment form.
  • Elementary Conversation Starters page (16 questions)
  • Middle and High School Conversation Starters page (17 questions)
  • Three different "real photo" conversation starters with questions included to encourage conversation
  • Fluency Data Graph - I use this to show progress from year to year during the IEP meeting. I keep a copy of the fluency data sheet in my student's folder. Each year, when I complete the informal assessment, I transfer the percentage of disfluency to this graph. The 0-50 will stand for the percent of disfluency documented on an informal evaluation. This way you can track progress or regression over time.
