THIS KIT INCLUDES:
QUARTERLY REPORT CARDS
- Math, reading and writing standards only.
- Editable sections include student name, date and a place to add/edit the sight words for each quarter.
- ----> All other graphics/text are flattened and secured. The quarterly standards and goals CANNOT be changed.
- Product preview shows exactly which standards are assessed in each quarter.
- Not every CCSS standard is included - please see the preview.
- This kit only assesses the major standards we test for report cards.
ADDITIONAL GOALS/STANDARDS - EDITABLE FORM
This page is where you can add any additional standards and goals your class is working on for each quarter - including science and social studies standards or other Math/ELA standards not addressed in the pre-made report card.
QUARTERLY ASSESSMENT KITS
These kits are pre-made and are not editable. They correlate perfectly with the report cards! All standards are common core aligned, and there are two or three mats for each quarter. Simply print, place in page protectors or laminate and you're ready to assess students! I use these to assess my students one on one, and the coordinating report cards make this go very quickly.
SOCIAL AND BEHAVIOR PROGRESS
The goals/standards are formatted and cannot be changed, but the place to add student name, date, and teacher comments are all editable so you can type in the form field. Students can receive an E,M,N for exceeds, meets or needs improvement for each goal.
QUARTERLY GOAL CHECKLISTS
This is a great handout for parents! I send home one with each report card. You can quickly check the standards students mastered for the quarter, and it also gives parents a heads-up as to what we will be working on each quarter. These are NOT editable as they correlate with the assessment kits and report cards.
STUDENT SELF-REFLECTION
I love having my students complete these each quarter! I keep a copy for their portfolios and I send one home each quarter with their report cards.
STUDENT DATA RECORD FORMS
An excellent resource for you to keep track of the standards students mastered and when they mastered them. If a student does not master a skill/standard during the quarter, you can quickly see which standards you need to retest the following quarter.

ITEMS YOU WILL NEED TO PROVIDE
The only items you'll need to provide are the sight word flash cards you're learning each quarter, manipulatives/counters (for 1:1 correspondence/counting standards and addition/subtraction) as well as your reading fluency passages for each quarter.
If you're testing additional standards, you may need to provide materials for those as well.
PLEASE READ BEFORE PURCHASING
This assessment kit tests very specific skills each quarter that fit with most schools/districts. Please do not purchase if these skills do not align with your standards and goals as many aspects of the kit cannot be edited or changed.
I did my best to make sure you could ADD any additional goals you need to, but the main goals and standards cannot be deleted or changed.
EDITABLE FORMS
Some pages in this kit are editable. Editing can ONLY be done using ADOBE 11 READER which is free to download. Other versions of Adobe do not work well for editing. Basic editing directions are included for your convenience.
