Enrollment Form for any facility, school, MDO, preschool

Blank Enrollment Form we use at our school. Can be used for any childcare facility, preschool, MDO (mother's day out), children's day out, church school, school, or other facility.

We created this document based on requirements of our state's child care facility licensing laws. It is comprehensive and meets all requirements for the state of OK, and probably most states.

Requests pertinent information such as child's name, birthday, allergies, parents and their contact information, emergency contact information, approval/disapproval for child to receive medical care or to be taken to emergency room, approved people for release of child, requested schedule (number of days and full day or half day), special needs, important information about a child, photo/video release, admission and withdrawal dates, etc...
