Description
When you download this resource you will receive a digital download link for the Google Forms. You can easily send the Google Form as is to parents and students, or you can edit the questions to suit you needs before sending.
You will also receive a PowerPoint document with a printable version for students and parents. This version also includes pages with just scales so you can add your own questions and decide what to write on each scale!
The questions I've included have helped me to keep my end goal in mind while teaching. For example, the questions "Did you feel safe in the classroom this year?" and "Did your teacher listen to you this year?" are included because these are things that I want my students to feel after spending a year with me!
This was a wonderful reflective process for me to create, and I definitely recommend using this as a powerful growth tool for your practice! :)
