The End of Year Parent Survey includes ten intentional questions to gather the thoughts of your parents. There is plenty of space for parents to add comments, the things they love and the things they think might benefit from being changed. Included in this product is a DIGITAL copy of the End of Year Parent Survey. This was created as a Google Form. You can EDIT it to meet your needs! I have included easy to follow step by step instructions on how to copy the Google Form, edit it to meet your needs and get the digital copy to parents via email, link or embed code.
The Teacher Report Card is a fun and creative way for students to rate, grade and comment on you as a teacher. It is the students turn to give you a grade! This product was created to print back and front and fold to make a 'card.' I have included a page that has blank subject areas for the teachers that may only teach 1-2 subject areas. My students LOVE that they get a chance to give me a grade at the end of the year! This is a great way to see your classroom and instruction from the eyes of your students.
