End of Year - Student Reflection & Feedback Survey
This end of the year student reflection survey is a great way to not only hear feedback from your students but also take some suggestions from your students'. Students will have the opportunity to reflect on the school year and let you know their opinions.
Students will share if they felt respected, safe, learned a lot, thought the homework policy was fair, thought the tests were fair, etc. In addition, students will then share some suggestions (if any) they may have for the next school year. I love completing this activity with my 6th grade students at the end of the year because I always learn at least one thing. They are fun to read and also provide helpful and honest feedback.

HOW TO USE:
No prep necessary! Simply copy the student reflection (two-sided) for each student. Explain how you would like students to choose between strongly agree, agree, no opinion, disagree, and strongly disagree. You may also choose to make the reflection anonymous.
